CLEVELAND -- How much money would it take to get you to lose weight and get healthy? Would you do it for a boost in your paycheck? About a third of American businesses offer some sort of incentive to encourage employees to get healthy.

Medical Mutual, the state's largest health insurance company, knows first-hand the cost of poor health.

For the last five years, they've rewarded employees with points for participating in wellness programs. At the end of each year, employees can then cash in for benefits, such as a premium reduction or a gift card.

Medical Mutual president and CEO Rick Chiricosta wanted to get in shape himself, so he challenged his employees to join him.

"I thought, if I went public with my need to do something, it would put pressure on me to make sure I did what I needed to do," Chiricosta says.

It started in January and the "Chiricosta Weight Loss Challenge" helped Jolynn Isaac lose 39 pounds, but she didn't do it for the $2,500 prize money.

"It's a big motivator to know that there were going to be a lot of other employees losing weight and wanting to lose weight and I'm a very competitive person," Isaac says.

The company's health risk assessment informed Kenitha Sims that she had hypertension. She lost 43 pounds, thanks to a company-paid Weight Watchers program and the boss's challenge. A chance for a cash prize helped keep her motivated.

"Personally, I think the benefit of the incentive helps me to lose weight," Sims says.

However, critics and psychologists say people are more motivated by losing their own money than by winning someone else's but that's not what they've seen at Medical Mutual.

"Rewarding is much more positive when you tend to go to a dis-incentive rather than a positive incentive. We see a lot of 'Big Brother' kind of feelings develop," says Paula Sauer, Medical Mutual VP of care management.

Chiricosta agrees negative reinforcements may work short-term but won't help morale.

"We believe healthy happy employees are better employees and take better care of our customers," he says.

Medical Mutual created a healthy cultural change and says their research shows incentive programs work. More than half of the employees are taking part in the weight-loss challenge.

It may also be why 85 percent of their 1.5 million customers use some type of wellness program offered through the insurance company.

Some doctors worry about overuse of weight-loss surgery: Others excited about its potential for moderately obese

KALAMAZOO — As gastric bypass and other bariatric surgeries are becoming more popular for weight loss and as minimally invasive surgical techniques are developed, some health professionals worry that the procedures may be overused.

Cooper is the medical director of the Medical Weight Loss Clinic, a Southfield-based business that has 34 locations in Michigan and northern Ohio, including one in the Kalamazoo area.

“Bariatric surgery is a last resort, as I see it,” said Cooper, who was trained as a bariatric physician but has not done bariatric surgery.

He was raising his concerns in response to a recent Los Angeles Times report saying that new, incisionless techniques that are now in human trials at major hospitals may end up broadening the use of bariatric surgery to people who are only moderately overweight or on the lower end of the obesity scale.

“I think 100 pounds overweight or more with co-morbidity (another health condition such as diabetes) is a good rule to follow,” Cooper said.

The clinics that Cooper oversees are devoted to medically supervised weight loss through diet, exercise and medication. “We do some referrals for bariatric surgery” when it’s appropriate, he said.

“Bariatric surgery does offer the best results of any weight-loss treatment available today,” Cooper said, “but it’s not without the possible recurrence of weight gain, and there are post-surgical issues” such as the risks of leakage, bowel obstruction and dumping syndrome, which is involuntary vomiting or defecation.

“The risks of bariatric surgery must be weighed against the risks of being morbidly obese (overweight by 100 pounds or more or with a body mass index of 40 or greater), diabetes, arthritic problems, heart problems, certain cancers,” Cooper said. “It’s appropriate for the right people.”

Divided opinions

Health professionals at major medical centers who were quoted in the L.A. Times report — and in a similar Chicago Tribune story — were about evenly divided on using bariatric surgery in those who are not extremely obese.

Some said the surgery is underused as a way to deal with Type 2 diabetes and other obesity-related health problems, while others said the surgery carries the risk of long-term complications and that diet changes, exercise and medication should be recommended instead for the overweight or moderately obese.

The L.A. Times noted that statistics from the American Society for Metabolic & Bariatric Surgery show bariatric surgery rates have doubled in the United States in the past six years, with 220,000 of the procedures done in 2008.

Dr. Stuart VersemanView full sizeCourtesy of Borgess Medical CenterDr. Stuart VersemanDiabetes, though, is on the rise, too, and studies show bariatric surgery is achieving good results in controlling that disease in patients with BMIs between 30 and 35, said Dr. Stuart Verseman, who has been medical director of bariatric surgery at Borgess Medical Center since October 2005 and does bariatric surgery at both Borgess and Bronson Methodist Hospital.

“They’re finding a marked improvement in their diabetes after bariatric surgery and a decreased usage of medication,” Verseman said.

An example of someone with a BMI of 30 would be a person who is 5-foot-9 inches tall and weighs 203 pounds. That person would be 35 pounds above the normal weight range for that height, according to the U.S. Centers for Disease Control and Prevention.

While that example might not strike some people as a case of obesity, Verseman said that “even people with BMIs of 30 are considered clinically obese.”

Verseman, following National Institutes of Health guidelines, does bariatric surgery only on those with a body mass index of 35 or more and certain related medical conditions or 40 or more without those conditions.

But he said he finds it “very exciting to think about offering that to a larger population, especially with the number of diabetics increasing astronomically every year.”

Dr. Alan Saber, who had been the main physician doing bariatric surgery at Bronson, left in November and now works at Case Western Reserve University Hospitals, in Cleveland.

Verseman, like Cooper, said bariatric surgery should never be the first choice for weight loss. But he is much more open to the potential of its use in the moderately obese who have diabetes, high blood pressure or other weight-related conditions.

Krista Hampton, a registered dietitian who is Bronson’s bariatric coordinator, sees the value of bariatric surgery for the extremely obese, but she expressed reservations about using it in people who are overweight or moderately obese.

“Coming from my background, I would want someone to exhaust all other weight-management options first,” she said.

Surgery and support

Bariatric surgery is on the rise at Borgess and Bronson, as it is nationwide. In the fiscal year ending June 30, 2004, there were 14 bariatric surgeries at Borgess. From July 2008 through June 2009, there were 187 at Borgess, and in the last six months of 2009 there were 110, according to Rebecca Blades, a registered nurse who leads the Borgess bariatric program.

Verseman said his bariatric surgeries have had a major-complications rate of 2 percent, while the national average is 3.5 percent.

Representatives of both hospitals emphasized that they carefully screen those interested in bariatric surgery and offer continued support to those who get the surgery, trying to help them avoid overeating and stay active.

Borgess, for example, offers support groups, both in person and online, and offers classes in active living, Blades said.

“None of these operations are cures,” Verseman said. “They’re tools to help patients. And it’s a team approach we take, involving a dietitian, a psychotherapist, the primary-care physician. Patients need to follow guidelines afterward to improve their medical conditions.”

They need to eat small meals and make sure they’re getting the right nutrients and enough exercise.

“If you start grazing, over time you can get a lot of calories in even after surgery,” Verseman said.

But if people getting bariatric surgery have to learn to make dietary and behavioral changes and stick with those changes to lose weight, why not instead work closely with people to support new eating habits and more exercise without doing bariatric surgery?

“I wouldn’t have somebody undergo bariatric surgery who has never attempted anything else before,” Verseman said. “Almost every patient (who undergoes bariatric surgery) has completed numerous diets” but has not succeeded in losing enough weight to improve their medical condition, he said.

“You don’t just wake up one morning and say, ‘I want to have gastric bypass surgery.’”

The surgery helps people feel full and therefore have less of an urge to eat, Verseman said. One type of surgery he does — a sleeve gastrectomy — takes out 75 percent of the stomach and gets rid of the hormone that causes hunger, he said.

Even without bariatric surgery, though, people can learn to ease feelings of intense hunger, Hampton said, by increasing the protein and fiber in their diets and decreasing the kinds of high-carbohydrate foods that cause blood sugar to spike and then drop.

She said some people who enter a weight-management program at Bronson with the idea of getting bariatric surgery decide not to have it once they learn to cook and eat differently, get more exercise and overcome some behavioral issues.

“We need to get back to the basics of how to cook and how to meal plan,” she said. “Convenience food is way too easy to get our hands on.”

Verseman expressed a similar sentiment but said he is glad he can help improve people’s lives through bariatric surgery.

“To see people get over medical conditions and get their lives back ... is really gratifying,” he said.

Caution advice for anti-obesity pill

CONTROVERSIAL fat-busting drug Reductil will continue to be sold in Malaysia -- for now.

The Health Ministry, in a statement yesterday, said the Drug Control Authority (DCA) had received information regarding suspension of products containing Sibutramine (the active ingredient in Reductil) by European medical regulators.

The decision was made based on the results of the Sibutramine Cardiovascular Outcome Trial (Scout), conducted by Reductil's maker Abbott, to evaluate cardiovascular safety in high-risk patients with a history of cardiovascular disease.

The six-year study, which involved 10,000 overweight and obese patients who had a history of heart disease and/or diabetes, indicated that about 11.4 per cent of those who took the drug had a heart attack or stroke compared with about 10 per cent who took a placebo.

In Malaysia, there have been a total of 35 adverse drug reports for Sibutramine to date. Four were related to cardiovascular problems. Three of them were of palpitations and one was of a non-fatal myocardial infarction.

"The DCA will instruct all Sibutramine product registration holders to circulate a 'Dear Health Care Professional' letter to all prescribers in Malaysia regarding this new information as well as adding the description of the Scout study in the product insert, to further strengthen its safety information," the ministry said.

It added that all nine drugs containing Sibutramine sold in Malaysia have warnings for patients with a history of coronary artery disease, congestive heart failure, tachycardia, peripheral arterial occlusive disease, arrhythmia or cerebrovascular disease and inadequately controlled hypertension.

The DCA will continue to monitor the situation and assess any new information regarding Sibutramine for further regulatory action.

The ministry's decision was described as "a sensible move" by National Heart Association vice-president Datuk Dr Azhari Rosman.

He said that though many clinical trials had taken place, they had their limitations.

He added that it "is good practice" to have feedback from many parties before deciding to yank medicines off shelves.

Reductil, according to its maker Abbott, is prescribed to over 15 million patients in 80 countries.

It works as a weight loss drug by blocking nerve cells that release and reabsorb the hormone serotonin, a chemical neurotransmitter in the brain that affects moods and appetite.

As the level of serotonin in the body rises, people feel fuller, eat less and, as a result, lose weight.

The Scout findings set alarm bells ringing in Europe and the United States.

Last Thursday, the European Medicines Agency conducted a safety review of medicines containing sibutramine and concluded that the risks of the medicines outweighed their benefits.

It recommended suspending marketing authorisations for sibutramine and removing drugs with the chemical from all markets in the European Union.

Medicines that contain sibutramine include Reductil, Reduxade and Zelium.

The same day the US Food and Drug Administration requested the manufacturer of the drug to add a new contraindication to the existing warning for the sibutramine drug label. Reductil is sold in the United States as Meridia.

The new warning will indicate that patients who have uncontrolled high blood pressure, heart arrhythmias, peripheral vascular disease, or those who have had a personal history of heart attack or stroke, should not use the drug for weight loss.

Australia's Therapeutic Goods Administration (TGA), on the other hand, confirmed that there would be no restrictions placed on the drug.

The drug has also been widely used in India since 1999. However, the Drug Controller General of India (DCGI) has now decided to take the matter to an expert group.

"If experts see a problem in the drug, the manufacturers would be asked to immediately stop its production," DCGI Surinder Singh told The Indian Express.

'Biggest Loser': Quick-fix diet is no fix

Helen Phillips tried every quick-fix diet known to mankind.

She ate cabbage soup for a week. She went on the vanilla ice cream and hot dog diet, and she popped diet pills like candy, hoping for that miracle body. The results? She'd lose weight, then gain it back.

"I was a yo-yo dieter for 13 years," said Phillips of Sterling Heights.

Eventually Phillips swelled up to 257 pounds and ended up on the national television show "The Biggest Loser." Now she does things the right way. The days of the quick fix are over. She is now a motivational speaker for Henry Ford Hospital and is working on changing the nutritional menu for Warren Consolidated Schools.

We met this weekend during the Curtis Granderson celebrity basketball game. Phillips is 140 pounds lighter, lost 54.47 percent of her body weight -- and making it up and down the court was not the hardest thing she's done. Phillips has run a marathon, won "The Biggest Loser" and has agreed to join our cause in promoting 40 days and 40 nights.

We are entering the final third of 40 days and 40 nights. Phillips recognizes the importance of our program and others that encourage good, healthy living. Phillips is intrigued by the concept and has agreed to share some of her experiences during our next phase of good living. (Next month we will begin 40 days and 40 nights of exercise.)

"Quick fixes do not work," she said. "It makes things worse. It is quick fixes that really messes you up and creates havoc with your metabolism."

The message is we must do things the right way. Phillips' stint on "The Biggest Loser" was excessive. She worked on her body for six to eight hours a day.

But when she came home, reality hit. The same lifestyle that caused her to gain weight was still there. She needed to work and take care of her family, and did not have endless hours to work on losing weight. "Nothing else changed. I changed," she said. "And I needed to introduce it to my friends. I wanted to come in and be accessible to everybody. You can do this at home. You won't lose like you did on "Biggest Loser."

"When you gain weight your self esteem gets lower," Phillips said. "You get tired and exhausted and you are always looking for a quick fix. It is not a diet. It is a lifestyle change. You can lose 50 pounds with a quick fix. Then what? What happens after that?"

What happens? You normally gain the weight right back.

Fruit and Weight Loss

Weight loss help just a seminar away

If you are trying to lose weight, but don’t know quite how to proceed you may want to attend a seminar Thursday held by Northeast Georgia Medical Center.

The Weight Loss Seminar will be held in the hospital’s Walters Auditorium from 6:30 p.m. until 8 p.m. Hospital staff will offer exercise and dieting tips, as well as discuss various surgical weight loss options.

“We wanted this seminar to be attractive to people in all categories: those who are trying to just lose 15 or 20 pounds, those who need to lose 50 or more pounds and those who are suffering from morbid obesity and need to lose 100 or more pounds as well as resolve co-morbidities,” said Hannah Day, NGMC Bariatric Weight Loss Center program coordinator.

“This seminar is part of our commitment as a program to help educated the community about the importance of weight loss as obesity contributes to so many health problems.”

According to Day, the diet and exercise tips that will be shared by registered dietitian Mindy Bell are important for everyone, no matter what their weight loss goals are.

“All of our speakers do a great job explaining that the information (that Bell) shares about nutrition and exercise is important for everyone, even those who undergo a surgical weight loss procedure,” said Day. “The procedures are just tools to help them in their weight loss quest. They still must eat right and exercise to lose weight and to keep it off long term.”

The different surgical options that will be discussed include banding, gastric bypass and sleeve gastrectomy. Although the goal of all the surgical procedures is the same, the level of invasiveness varies between the different options.

“We feel that these public outreach meetings are vitally important in getting the message out to our community regarding the epidemic of obesity. This is one of the fastest growing health problems in the U.S. and the industrialized world,” said Dr. Charles D. Proctor Jr., who will participate in the seminar.

“Our purpose is to educate the community about this disease and to present some options, both surgical and nonsurgical, in the management of it. Many people are understandably reluctant to pursue surgical weight loss but everyone needs to be educated about the risks and benefits of all options so that they can make an informed decision.”

The seminar is free and open to the public, but participants are asked to register in advance for the event, which includes a complimentary, light dinner.

Surprise Exit In 'The Biggest Loser'

A week ago, Patti and Stephanie looked like one of the teams to beat this season of "The Biggest Loser." But after a disappointing weigh-in, the mother-daughter team found themselves in the hotseat to go home. Here's how it went down.

The contestants recieved a bunch of eye-opening news when Dr. H makes his routine visit to the show. Once again he tells the contestants that they are the most unhealthy ever. (Unlike shows like American Idol, when this show makes claims about something being the biggest or worst, they've always got the statistics to back it up.)

Jillian sat down with mother and son team Cheryl and Daris to discuss their eating habits. Jillian unveils the food that the two eat on a regular basis, and it's basically a tour of Taco Bell. Jillian attempts to eat the mounds of food on the table, but dramatically spits it out into the garbage. (Come on, Jillian! I know it didn't taste that bad.)

The week's challenge involves the contestants having to carry balls across a pool by walking across a 40-foot beam. Maria laments to the camera that she is afraid of the water. (A sign of things to come?)

Lance and Melissa won the challenge, which also meant that they recieved an immunity. Maria and Michael were the last team, which meant they recieved a two pound penalty during the final weigh in. More important was the way they lost. Maria, who I already told you is scared of water, lost her balance and fell to the ground face first in order to avoid the water. The result was a bloody mess, a black eye, and a fractured finger.

Despite the injury, Michael and Maria trained hard and lost a combined 21 pounds. It was enough to place them ahead of Patti and Stephanie, who only lost a combined 7 pounds despite losing 41 pounds the first episode. (Their effort in the first week made them a team to beat. I was truly surprised to find them in last place.)

It should also be noted that Melissa actually gained a pound this week. It was totally strategy because her team had immunity this week. It will likely mean that she will have more to lose next week when there is no guarantee of immunity.

Patti and Stephanie convince the others to vote off Patti because she claims to have a better support system at home.

In the update segment of the show, the audience learns that Patti has lost 43 pounds since the show. It appears that she has made the most of her experience on the show.

Milk wins in weight war

Teenagers wanting to maintain a healthy body weight should forget about diet soft drinks, and go for milk instead according to new research .

Researchers from the University of Minnesota's School of Public Health used data from Project EAT (Eating Among Teens), a five-year study of eating patterns among 2294 adolescents, to examine the relationship between beverage intake and change in Body Mass Index.

They found that teenagers who drank milk had a significantly lower increase in BMI than those who rarely or never consumed milk.

This is consistent with a growing body of scientific research which shows a connection between dairy consumption and weight management in adults.

Some studies show that people who consume at least the government's recommended two to three servings of low or reduced-fat dairy each day were more successful with weight management and weight loss than those who don't.

Interestingly, diet soft drink consumption was associated with BMI gain.

The authors suggested that this may be due to the relationship between diet soft drink consumption and dieting behaviours.

They noted that dieting has been shown to be associated with weight gain in previous studies with adolescents.

The study's authors concluded that interventions with adolescents should promote the consumption of low-fat milk and decrease the availability of sugar sweetened beverages in school and home settings.

Dairy Australia's dietitian Glenys Kerrins said unfortunately many Australian teenagers and young women cut back on dairy when they were watching their weight but this research showed that that was the last thing they should do.

Her advice for managing weight was to ``cut out junk foods, exercise more, and focus on eating nutrient-rich foods, including three serves of dairy, every day''.
